    iCADEMY ONLINE REGISTRATION

    POLICIES & PROCEDURES

    1. RELEASE OF LIABILITY
    I waive the right to any legal action for injury or property loss sustained on school property resulting from normal classroom activity or any other activity conducted by the student before, during, or after lesson time. I give Avalon School of the Arts permission to use my/my child's photograph in print or website publications. 
    2. TUITION
    All payments are processed by pre-authorized credit cards. Registration fee is $25 for new students and re-enrollment. Tuition for iCademy Students is $210.00 per semester. Registration & Semester Tuition is charged upon registration. (Total $235.00) Semesters are from September 3, 2024 - January 12, 2025 & January 13, 2025 - May 25, 2025. Tuition is never prorated for missed lessons. Tuition does not include any retail teaching materials such as books or instruments. A receipt will be emailed to the email address on file in your account. Charges will appear on your bank statement as Avalon School of the Arts.
    3. NSF PAYMENTS & PAYMENT DENIALS
    I hereby authorize Avalon School of the Arts to electronically charge my credit card or bank account for the amount of any NSF or payment denial transactions plus a $25 NSF/Payment Denial fee. 
    ​4. MAKEUP LESSONS
    The most important thing when learning a musical instrument is consistent weekly engagement. If the student is absent for a lesson, the teacher will post a detailed personalized make-up lesson to the student’s online journal during their normally scheduled lesson time with video, text, and attachments.  For any lessons missed by the teacher, a makeup lesson will be scheduled or a video lesson will be posted to the student's online journal. This ensures students (and parents) remain engaged and informed, even when they miss. We do not issue credits for student absences, even when prior notification is given. 
    ​5. SEMESTER COMMITTMENT
    I understand that I have committed my student to attend the full semester of lessons and that all payments are non-refundable.
    ​6. INCLEMENT WEATHER POLICY
    In the event of inclement weather (Avalon School of the Arts observes West Ottawa Public Schools inclement weather days) lessons will be held online that day (Zoom, Skype, or FaceTime). If the student is unable to do an online lesson, a personalized video lesson will be posted in the students’ online journal.  If roads are clear we will notify students that we are open by 1pm. 
    ​7. MEDICAL EMERGENCIES
    I give Avalon School of the Arts, it's owners and employees permission to seek medical treatment for the student in the event that they are unable to reach a parent or guardian. I hereby declare any physical/mental problems, restrictions, or conditions and/or declare the student to be in good physical and mental health. 
    8. CONTACT INFORMATION
    I understand that it is my responsibility to have accurate contact and billing information on file with Avalon School of the Arts at all times. Billing information, mailing address, phone numbers and email addresses can be updated in person at the front desk or over the phone (616) 294-1483. 
    ​9. BEHAVIOR EXPECTATIONS
    Students are expected to be well-behaved and respectful toward students and teachers both in lessons and in common areas. Students who are repeatedly disruptive and disrespectful, or whose behavior is a danger to themselves or others, may be asked to leave and have their enrollment revoked. 

    AUTHORIZATION AGREEMENT FOR PAYMENT

    Registration fee is $25 for new students and re-enrollment. Tuition for iCademy Students is $210.00 per semester. Registration & Semester Tuition is charged upon registration. (Total $235.00)
    (I/We) do hereby authorize Avalon School of the Arts LLC, hereinafter named the COMPANY, to initiate recurring (debit or credit) entries to (my/our) (Credit Card Account) as indicated and named below as the depository financial institutions, hereafter named FINANCIAL INSTITUTION. (I/We) acknowledge that the origination of ACH transactions to (my/our) account must comply with the provisions of U.S. law. Furthermore, if any such debit(s) should be returned NSF, (I/we) authorize the COMPANY to collect such debit(s) by electronic debit and subsequently collect a returned debit NSF fee of $25.00 per item by electronic debit from my account identified below, and authorize all of the above as evidenced by my signature below.

    This authorization is to remain in full force and effect until the COMPANY has received written notification from me (or either of us) of its termination in such time and in such manner as to afford the COMPANY and FINANCIAL INSTITUTION a reasonable opportunity to act on it.
    I have read and understand the Avalon Music Academy of Holland policies and procedures and payment authorization agreement and agree to abide by them.
